Presentation

Bilateral cervical and pelvic painless palpable mass on physical exam.

Patient Data

Age: 70 years
Gender: Female

Multiple para-aortocaval and mesenteric enlarged lymph nodes are seen that surround but do not occlude the mesenteric vessels and displace bowel loops.

Additionally, multiple enlarged lymph nodes are present in bilateral para-iliac regions that compress the urinary bladder and displace the bowel loops. Several inguinal enlarged lymph nodes are also evident on both sides.

The spleen is markedly enlarged, and its cephalocaudal height measured 215 mm. 

In the imaged portion of the lower thorax, mediastinal enlarged lymph nodes are seen. Additionally, mild to moderate pleural effusion is evident bilaterally, accompanied by mild adjacent lung collapse.

Multiple mediastinal, para-aortocaval, mesenteric, para-iliac and inguinal lymphadenopathy with huge splenomegaly in a pathology-proven case of Hodgkin lymphoma.
