Extensive bilateral (right greater than left) streaky interstitial pulmonary opacities with airspace opacification at the right base. Features are non-specific.

48 hours later, there has been an improvement in appearances with reduction in the degree of basal opacification.
Case Discussion
Appearances here are non-specific and gathering of appropriate history is key. Raised inflammatory markers were present and following treatment with antibiotics, ventilation was reduced and the patient was extubated on day 4 and made an uneventful recovery.
