Presentation

Patient who underwent heart transplantation three months earlier on immuno-suppressive therapy. She presents with occipital pressure ulcers and sepsis.

Patient Data

Age: 40 years
Gender: Female

Beneath the soft tissue wound in the posterior scalp, a moth-eaten appearance of the external table of the occipital bone is seen.

Case Discussion

This finding of the occipital bone moth-eaten appearance, in the clinical context, is highly suggestive of early osteomyelitis. Occipital bone osteomyelitis secondary to pressure ulcer is an uncommon complication that should be taken into account in long-term bedridden patients with pressure ulcers.
