Invasive ductal adenocarcinoma at time of diagnosis.
- a hypodense mass is seen at pancreatic head
- dilated CBD, gallbladder and pancreatic duct
- internal biliary drain along CBD till the duodenum
- mild pneumobilia
- few locoregional lymph nodes

Histology:
- pancreas: invasive ductal adenocarcinoma, moderately differentiated
- gallbladder: mild chronic cholecystitis; negative for malignancy
