Thick lined, loculated radio-opacity in right hemithorax, with an air-fluid level, loss of silhouette with the right dome of diaphragm. The right cardiac border can be appreciated.

CT confirms collection in the right pleural space with thickened pleura. Also noted are consolidation in the right lower lobe and calcified mediastinal nodes.

Imaging features are consistent with pyopneumothorax.
ICD was done using the thick bore tube and yellowish colored pus drained.
