Gradual onset of right-sided mild scrotal heaviness.

There are multiple small cystic/tubular anechoic structures that replace and enlarge the right testicular mediastinum. No mass.
There was no abnormal vascularity in either testicle.
Normal left testis.
The geographic shape, lack of mass effect, and lack of internal flow of the mentioned cystic/tubuar anechoic structures are consistent with tubular ectasia of rete testis.
Cystic ectasia of the rete testis must be differentiated from other benign intratesticular lesions, notably cystic dysplasia and intratesticular varicocele. Furthermore, as it is mostly asymptomatic, it should be distinguished from the rare papillary adenocarcinoma of the rete testis, in which a solid mass in the rete testis should be evident.
