Pelvic pain and fever.

Images reveal a midline pelvic collection with non-dependent air pockets and an enhancing rim.

There is an irregular calcified body to the right of midline within the collection, representing an appendicolith.

Case Discussion

Patient went to surgery and the diagnosis of ruptured appendix was confirmed.

Final diagnosis: acute appendicitis - perforated with pelvic collection.
