Presentation

Outpatient presentation with abdominal distension and a history of chronic hepatitis. An ultrasound is requested.

Ultrasound image shows shear wave elastography of the liver. The stiffness value is 19.2 kPa. Liver parenchyma is coarse and nodular with Irregular margins. Mild ascites is also seen.

Case Discussion

Shear wave elastography is a valuable additional ultrasound tool to assess the liver. Shear wave elastography is performed in fasting status.

In this case, the average stiffness score is 19.2 kPa. This range is diagnostic of F4-stage fibrosis compatible with cirrhosis. As values are higher than 17 kPa, the patient is at risk of clinically significant portal hypertension (CSPH) and hepatocellular carcinoma (HCC).
