The undulating fascia sign is a radiological sign described in MRI of the lower limbs, typically, but not exclusively, in sporadic inclusion body myositis.
The undulating fascia sign refers to the appearance of the fascia between the vastus intermedius and vastus lateralis muscles on T1 sequences of MRI of the proximal lower limbs 1-4. In certain diseased states, these muscles are severely atrophied and infiltrated by fat, resulting in the fascia between them taking an abnormal saggy, wavy or undulating morphology 1-4.
This sign has been described in:
sporadic inclusion body myositis: described in 75% of patients in one study 1
other idiopathic inflammatory myopathies with anterior compartment (quadriceps) muscle atrophy 4
anoctamin-5 (ANO5) related muscle disease 3
