Diagnosis certain

Presentation

The patient tripped and fell onto broken glass. She presents with an open right knee wound.

Patient Data

Age: 4 years
Gender: Female
x-ray

There is a well-defined, rectangular, radiopaque, foreign body embedded with the prepatellar soft tissues at the inferior margin of the right patella. There is periarticular soft tissue swelling. There are no post-traumatic bony abnormalities.

Case Discussion

Penetrating glass fragments although radiopaque can be challenging to identify on radiographs, especially if tiny. In this instance, the size and density of the glass shard make it easily identifiable at the inferior margin of the right patella. Tangential and multiple views are always recommended to resolve foreign body localization and exclude surface or overlying artefacts.
