Markedly dilated stomach and proximal duodenum (first part). Shelf-like structure is seen at the junction of the first and second parts of the duodenum, suggestive of a duodenal web. Collapsed second, third and 4th parts of the duodenum.

Case Discussion

Duodenal web refers to types of congenital duodenal obstruction of the proximal duodenum, either with an intraluminal duodenal diverticulum or a duodenal web. In this case, there is a perforated duodenal web with a central opening that exerts partial chronic obstruction of the first part of the duodenum. Management could be with repeated endoscopic balloon dilatation or surgically.
