Diagnosis almost certain

Presentation

Recent onset of ataxia with dizziness and vertigo.

Patient Data

Age: 25 years
Gender: Female

Areas of low signal on T1, high signal on T2, and FLAIR with restricted diffusion involving the cerebellar hemispheres (mainly the left), brainstem, vermis, right thalamus, and right medial temporal lobe. No hemorrhagic transformation was seen on the GE sequence. No significant mass effect is seen.

Occlusion of the distal portion of the basilar artery is noted on the MRA.
