Megalencephalic leukoencephalopathy with subcortical cysts

Diagnosis probable

Presentation

Headache and other vague, nonspecific complaints.

Patient Data

Age: 10 years
Gender: Male

Nearly symmetrical, extensive T2/FLAIR hyperintensities are seen in bilateral cerebral parenchymal white matter extending to subcortical U fibers at places with cystic changes in bilateral anterior temporal lobes.

Hyperintensities are also seen involving the posterior limbs of both internal and external capsules, with subtle hyperintensities in bilateral cerebral peduncles, brainstem, and bilateral middle cerebellar peduncles.

Case Discussion

Overall findings suggest megalencephalic leukoencephalopathy with subcortical cysts.

The patient belongs to the 'Agarwal' community of India, which is predisposed to this disease.
