Patient Data

Age: 75 years
Gender: Male

Chronic L1 crush fracture. Chronic L4 superior endplate infraction.

Individual review of the following intervertebral levels has been performed:

L2-3: Mild anterolisthesis. Circumferential disc bulge and severe bilateral facet joint arthropathy. A low T2, intermediate T1 signal mass arising from the right facet joint with central high T2 signal. Moderate spinal canal stenosis. Marked narrowing of the right subarticular zone with compression of the traversing right L3 nerve root, and mild bilateral neural exit foraminal stenosis.

Degenerative changes at the other spinal levels without high-grade stenosis.

Rim calcified mass arising from the right L2-3 facet joint containing gas. Severe bilateral L2-3 facet joint arthropathy with intra-articular gas.

Case Discussion

This is a case of degenerative lumbar canal stenosis with a right juxtafacet cyst that compresses the traversing nerve root. The findings on MRI are suggestive of a juxtafacet cyst given the location and signal characteristics and confirmed on CT, particularly given the gas within the cyst.
