Rugger jersey spine (CT)

Presentation

End-stage renal disease with secondary hyperparathyroidism. CT done for pre-transplant work-up.

Patient Data

Age: 25 years
Gender: Male
ct

Sclerotic bands seen in the superior and inferior endplates of the visualized spine with relative band of lucency centrally giving rise to a striped appearance (rugger jersey spine).

Bilateral atrophic kidneys with tiny renal calculi. No hydronephrosis or hydroureter.

Case Discussion

The rugger jersey spine sign (which looks like stripes of a rugby jersey) is usually seen in secondary hyperparathyroidism due to excessive osteoid deposition at the vertebral endplates. This patient had renal osteodystrophy due to end-stage chronic kidney disease.

Similar appearance of the spine may be seen in patients with osteopetrosis, however the demarcation between the sclerotic and lucent bands are more well-defined, therefore being termed as "sandwich vertebrae".
