Spinal epidural lipomatosis

Presentation

Chronic low backache.

Patient Data

Age: 25 years
Gender: Male

Excessive accumulation of fat within the epidural space at L5-S1 compressing the thecal sac which is narrowed with a "Y" shape configuration on the axial T2 sequence.

The anterior CSF space is moderately obliterated and some of the cauda equina are aggregated (impossible to visually separate) suggestive of moderate spinal stenosis (grade 2).

Case Discussion

MRI features of epidural lipomatosis compressing the thecal sac at L5-S1 with a "Y" shape configuration in a patient with moderate spinal canal stenosis (grade 2), according to lumbar spinal stenosis (grading).

The epidural lipomatosis involves mainly the lower lumbar and/or lower thoracic region.  Compression of the thecal sac may be symptomatic with a combination of pain, radicular symptoms, weakness and anesthesia.
