Posterior reversible encephalopathy syndrome (PRES)

Diagnosis probable

Presentation

The patient presented 10 days after delivery with a new onset of poorly controlled hypertension (up to 170/90), and a severe headache.

Patient Data

Age: 25 years
Gender: Female

There are few focal ill-defined heterogeneous hypodense areas seen in the left parietal and occipital lobes, the largest is seen at the left paramedian of the parietal region, measuring about 2.5 x 2 cm.

At that time, MRI was requested for further evaluation of these lesions seen in non-contrasted brain CT.

Bilateral subcortical white matter T2/FLAIR hyperintensities that show posterior predominance in their distribution, more pronounced in the left parieto-occipital lobes. They do not show mass effect or restricted diffusion. T2 shine through is seen at the DWI

These features are suggestive of posterior reversible encephalopathy syndrome (PRES).

Case Discussion

This patient presented with poorly controlled hypertension and severe headache, a CT scan was done followed by an MRI, which showed features suggestive of posterior reversible encephalopathy syndrome (PRES).

Unfortunately, further clinical or imaging follow-up is not available for this case.
