Primary intraventricular hemorrhage

Presentation

Confusion and falls.

Patient Data

Age: 60 years
Gender: Male

intraventricular hemorrhage, which is predominately filling the posterior right body, right trigone and right temporal horn with only a small volume in the left. No hydrocephalus.

Focal left mid brain hypodensity is likely an old infarct. Patchy deep white matter and periventricular hypoattenuation is most given the chronic small vessel ischemic change. 

Case Discussion

The patient proceeded to DSA, which did not show an abnormality. At presentation, the patient's systolic blood pressure was 190 mmHg, and the patient had a long history of poorly controlled hypertension despite multiple antihypertensive medications.
