Presentation

Productive cough, dyspnea, elevated CRP and WBC.

Patient Data

A circumscribed consolidation is visible medially in the right lower lung zone. The right heart border is not effaced, however, the silhouette sign is not a foolproof feature.  A slightly elevated right hilum is also apparent.

Case Discussion

Considering the clinical picture as well as the lab results, the chest x-ray findings are in line with right middle lobe pneumonia.
