Presentation
The patient complained of pelvic pain and prolonged and heavy menstrual bleeding. Ultrasound showed a large pelvic mass arising from the uterus.

A well defined T2 and STIR hypointense mass located at the fundus of the uterus in keeping with a uterine fibroid.
The junctional zones are diffusely thickened with numerous T2 hyperintensities interspersed cystic spaces are suggestive of adenomyosis.
