Anterior temporal lobe perivascular space

Diagnosis almost certain

Presentation

Headache and vertigo.

Patient Data

Age: 45 years
Gender: Female

Cystic lesion, measuring approx. 7.5 x 5.5 x 6.7mm (AP x TD x CC) in left anterior temporal lobe with mild perilesional edema.

Suspicious curvilinear hyperintensities are seen along convexity sulci of bilateral parietooccipital lobes, possibly artifactual. However, contrast enhanced MRI (not performed) would be more informative to completely rule out meningitis.

Case Discussion

This is a typical anterior temporal lobe perivascular space on left side with mild perilesional edema and should not be mistaken for a tumor or abscess.
