Age: 60 years
Gender: Female

Comminuted fracture of the distal radius with extension to the intra-articular surface along the dorsal aspect. Avulsion fracture of the styloid process of the ulna is also observed.

Case Discussion

Fracture-dislocation or subluxation of the wrist in which the dorsal or palmar edge (most frequently the latter) of the distal radius scrolls along with the hand and the carpus. That is, the carpus is dislocated and fractures the rim of the radius (dorsal or fly).

The mechanism is a fall on the wrist in dorsiflexion and fixed pronation. Usually are unstable and require open reduction and internal fixation with a cancellous screw or an Ellis backing plate.
It's called a Barton fracture when the fragment is marginal dorsal whereas when it is marginal fly is called Barton inverted and coincides with Smith fracture type II.
