Presentation

Left loin pain, CT assessment of the urinary tract.

Patient Data

Age: 45 years
Gender: Female

CT study shows pneumoperitoneum with free intraperitoneal gas is seen at the left upper abdomen, peri splenic region, and left hypochondrium. The splenic flexure of the left colon shows intramural gas bubbles (cystoides). No evidence of free intraperitoneal fluid collection or fat blurring.

No urinary stones or obstruction.

Case Discussion

Pneumoperitoneum is not always a serious finding, it is occasionally caused by benign conditions like benign pneumatosis coli. Clinical history and negative CT signs of peritonitis are diagnostic keys.

Checking the presence of pneumoperitoneum is advised to be done routinely in CT abdominal studies using the lung window that is so helpful.

This patient followed a conservative treatment with relief of symptoms within a few days.
