Crossed doubled patellar tendon

Presentation

Left knee recurrent dislocation

Patient Data

Age: 45 years
Gender: Female

Normal position of the patella within the trochlear groove is noted. No evidence of patella alta or trochlear dysplasia is detected. A crossed double patellar tendon is noted. The medial patellofemoral ligament is intact. No evidence of meniscal tear is detected. The anterior cruciate, posterior cruciate and collateral ligaments as well as the quadriceps tendon are intact. Mild joint effusion is noted.

Case Discussion

Double patellar tendon is a very rare anatomical variant and may be linked to chronic knee pain. There are two types of double patellar tendon: 1) Uncrossed type and 2) Crossed type as we can see in our case, where the superficial bundle of the tendon arises from the inferior patellar pole medial to the deep bundle, then it crosses the deep bundle from medial to lateral to insert on the tibial tuberosity lateral to the insertion of the deep bundle.
