Presentation

Right hip pain

Patient Data

Age: 40 years
Gender: Male

L3-4 right extraforaminal disc protrusion, encroaching upon the right L3 exit nerve root.

L3-4 posterior central disc protrusion, indenting the ventral thecal aspect and partially encroaching upon lateral recesses.

L4-5 diffuse posterior disc bulge, effacing ventral thecal aspect and partially encroaching upon exit foramina.

L5-S1 diffuse posterior disc bulge with left posterolateral disc protrusion that shows minimal caudal migration, encroaching upon the left lateral recess and bilateral exit foramina.

Case Discussion

Extraforaminal disc protrusion is an uncommon form of disc herniation, that is easily overlooked as it doesn't impinge upon the spinal canal. It can compress the exiting nerve root at the same level mimicking the effect of a posterolateral disc of the above level.
