First metatarsal and great toe osteomyelitis and septic arthritis of the first MTP joint

Diagnosis almost certain

Presentation

Type 2 diabetic from 15 years ago referred with fever and ulcer along with the medial aspect of the forefoot.

Patient Data

Age: 55 years
Gender: Male

Mild effusion is seen at the first metatarsophalangeal joint.

The bone marrow of the first metatarsal, great toe proximal phalanx base, and sesamoid bones show abnormal signal changes (low on T1 and high on T2/PDfs).

Skin ulcer with edema and thickening of subcutaneous tissue is seen at the medial aspect of the first

metatarsophalangeal joint.

Soft tissue edema and thickening are also seen along the dorsal and plantar aspects of the foot.

Case Discussion

Imaging findings are suggestive of the first metatarsal, great toe proximal phalanx and sesamoid bones osteomyelitis with metatarsophalangeal joint septic arthritis.
