Updates to Case Attributes

Title was changed:
Hepatic metastases - MRI and PET scanning
Presentation was changed:
PHPast history colorectal cancer. Known liver metastasis - assessment for hepatic resection.
Age changed from 70 to 70 years.
Body was added:

MRI especially with delayed post-contrast imaging using a hepatocyte agent (Primovist) is more sensitive than PET for hepatic metastases especially smaller masses and should be performed (as well as PET for extra-hepatic disease) prior to any planned resection so as to provide the surgeon with the best road map.

Updates to Study Attributes

Findings was changed:

FDG PET/CT of the upper abdomen showing intense isotope activity in the dome of the right hepatic lobe consistent with a metastasis.

Updates to Study Attributes

Findings was changed:

MRI especially with delayed post contrast imaging using an hepatocyte agent (Primovist)Multiple subtle lesions in the right lobe. On the arterial phase dynamic series, the PET avid lesion is more sensitiveclearly seen but there are two further lesions, i.e. in the gall bladder fossa and posterior right lobe. These are easily seen on the PET/CT scan as any higher isotope accumulation than PET for hepatic metastases especially smaller masses, and should be performed (as well as PET for extra-hepatic disease)  prior to any planned resection so as to provide the surgeon with the best road mapbackground liver.

Images Changes:

Image MRI (T1 in-phase) ( update )

Description was removed:
Multiple subtle lesions in right lobe

Image MRI (T1 C+) ( update )

Description was removed:
On this arterial phase dynamic series the PET +ve lesion is clearly seen but there are 2 further lesions ie in the gall bladder fossa and posterior right lobe. These are easily seen on the PET/CT scan as any higher isotope accumulation than the background liver.

Image MRI (late phase (~30 mins)) ( update )

Description was removed:
The most useful sequence is this delayed series where hepatocytes have taken up the contrast agent leaving the metastases as well defined "filling defects"
