Hoffa fat pad ganglion cyst

Presentation

Pain and discomfort.

Patient Data

Age: 30 years
mri

Large lobulated cystic lesion with internal septations, measuring approx. 20 x 37 x 25mm (AP x TD x CC) is seen in central, lateral aspects of Hoffa’s fat pad. Lesion is indenting articular cartilage of lateral femoral condyle, intercondylar region of femur superiorly; both tibial condyles inferiorly; patellar tendon anteriorly; anterior horn of lateral meniscus posteriorly and iliotibial band laterally. Surrounding soft tissue edema is seen. No obvious communication with anterior horn of lateral meniscus is seen in present scan. No obvious articular cartilage or bone erosions seen.

Small Baker’s cyst, measuring approx. 25 x 17mm is seen.

Edema is seen between iliotibial band and lateral femoral condyle.

Small suspicious vertical meniscal tear of posterior horn of medial meniscus seen, reaching tibial articular surface.
