Presentation

Long standing swelling and chronic pain of the left knee following trauma.

Patient Data

Age: 10 years
Gender: Male
mri

Multiple intra-articular frond-like lesions, predominantly involving the anterior synovium, demonstrated high signal intensity on T1, low signal on all fat saturation sequences, chemical shift artifact in the fat (lesions) - fluid (effusion) interface on GRE sequence, showed no enhancement, and with an associated moderate joint effusion. Multiple enlarged popliteal lymph nodes.

Case Discussion

Findings are typical of lipoma arborescens of the left knee joint. Lipoma arborescens is a rare condition with lipomatous lesions affecting the synovial lining of the joint and bursa. The condition is commonly seen in the 5th to 7th decades of life, but is also seen in children, as in this case.
