Presentation
Postictal seizure condition, altered mental status, erratic breathing, confusion, and urine incontinence. The patient is 9 months postpartum, with a history of gestational hypertension, polyhydramnios, and postpartum depression. On presentation, the patient was drowsy and unresponsive.
Patient Data
Large heterogeneous mass within the anterior left frontal lobe, which contains an internal multiseptated/cystic area and surrounding T2 hyperintensity and heterogeneous enhancement.
There is restricted diffusion and partial effacement of the frontal horn of the left lateral ventricle.
Right midline shift is also noted.
HISTOPATHOLOGY:
A biopsy of the brain lesion later revealed atypical lymphoid processes, which were confirmed to be oligodendroglioma (Grade 3) with an IDH2 mutation.
Case Discussion
Functional MRI was performed, revealing language involvement in the posterior lateral margins of the mass. Surgical resection was initially withheld and later chemotherapy was opted for.
