Presentation
Chronic pain over the anterior knee, with on and off swelling. Previously very athletic.
Patient Data
The lateral image is slightly overexposed making it difficult to assess the anterior soft tissue planes clearly, however obviously apparent are multiple, well corticated, smooth bone fragments of varying sizes at the tibial tuberosity.
Sharp patellar tendon margins are also not visible at this level.
Case Discussion
In Osgood-Schlatter disease, a patient may present after several years of recurrent symptoms of anterior knee pain and swelling. Commonly for this condition, there is also a history of a very athletic/ active adolescence.
If not managed early, the condition can progress to several sequelae including formation of ossicles within the distal patella tendon, pseudoarthrosis between a patella tendon ossicle, and the tibial tuberosity as well as pseudo-intra-articular loose bodies.
