Presentation

Presents with intermenstrual bleeding. Past history of left salpingo-ophorectomy.

Patient Data

Age: 35 years
Gender: Female
ultrasound

Unilocular solid right ovarian mass measuring 4.8cm with papillary projections (fewer than 4) and areas of acoustic shadowing.

Normal uterus with thin endometrium. 

Previous left oophorectomy, with no abnormality seen in the left adnexa.

Case Discussion

Unilocular solid ovarian mass with papillary projections in a young female patient.

Based on IOTA simple rules, this mass has 1 B feature (acoustic shadows) and no M features. 

ADNEX score: 96% chance of benign tumor. 

O-RADS category 4: Intermediate (10-50% chance of malignancy)

History withheld initially: contralateral oophorectomy was for a benign cystadenofibroma.  

Histologic diagnosis of this lesion: Cystadenofibroma.
