Presentation

Ring finger paronychia following injury whilst building a wire fence.

Patient Data

Age: 45 years
Gender: Male
x-ray

Focal soft tissue swelling of the distal fourth finger, particular at the dorsal nail bed. No foreign body material identified. No gaseous lucency. The bones are normal.

Case Discussion

Paronychia represents infection of the nail bed and is usually due to trauma or nail manipulation but can be spontaneous. In children and certain adults, it can be caused by nail-biting or finger sucking.

Xray is seldom required for diagnosis except when a foreign body requires exclusion.

Prompt incision and drainage is required to relieve the build-up of pus and extension deep to the nail. Occasionally infection can spread into the finger and hand via the tendon sheaths.
