Perforated gallbladder

Presentation

Severe right upper quadrant pain with tenderness.

Patient Data

Age: 45 years
Gender: Male

Distended gallbladder with thickened wall and minimal pericholecystic fluid. It contains multiple stones largest measuring 12 mm as well as thick sludge with septations. These findings are in keeping with acute calcular cholecystitis.

Diffuse thickening of the gallbladder wall and interrupted mucosa represent a defect in the medial wall of the body extending to the fundus.

It is surrounded by mild pericholecystic fluid collection, adjacent fat stranding, and two small stones are seen at the gallbladder neck. According to the Niemeir classification, this is consistent with a type I acute free perforation of the gallbladder.

Large simple renal cyst in the right upper pole with multiple bilateral small renal cysts.

Case Discussion

This case is an example of a type I perforated gallbladder most likely secondary to acute calcular cholecystitis. The patient underwent an emergency laparoscopic cholecystectomy.
