Perineural tumor spread along the greater occipital nerve

Presentation

Recurrence of previously resected cutaneous SCC over right occiput.

Patient Data

Age: 40 years
Gender: Male

MRI demonstrates an enhancing, irregular, cutaneous and subcutaneous mass in the occipital region.  An enhancing cylindrical structure extends anteriorly, passing along the superficial surface of the obliquus capitis inferior and terminating at the C2 vertebral foramen, consistent with the course of the greater occipital nerve.

Case Discussion

Radical excision involved C1-C2 laminectomy and confirmed intraneural SCC along the greater occipital nerve.
