Increase radiolucency in the right upper lobe.
No consolidation or collapse.
Heart and pleural spaces are clear.

Progressive radiolucency in both upper lobes with a paucity of pulmonary vessels.
Heart size normal.
Case Discussion
Pulmonary emphysema is strongly related with smoking.
Although the patient is a young smoker, he had progressive emphysematous changes within the lungs which were more eminent on the follow up radiograph. Smoking cessation was advised in this case.
