Pulmonary sequestration - intralobar

Case contributed by Ammar Haouimi
Diagnosis almost certain

Presentation

Sent by his cardiologist for an abnormal chest x-ray (not available).

Patient Data

Age: 35 years
Gender: Male

The scout view shows a hyperlucent left lower lobe with mediastinal shift to the right.

The CT scan demonstrates a hyperinflated hyperlucent left lower lobe sparing the apical segment. It has no connection with the bronchial tree, surrounded by normal lung tissue without its own pleural covering.

It has a systemic arterial supply arising from the descending aorta below the diaphragm and the venous drainage is via the pulmonary venous system (left inferior pulmonary vein).

Case Discussion

CT features of an intralobar pulmonary sequestration (incidental finding) which is the commoner type of pulmonary sequestration with a strong predilection towards the lower lobes (predominantly left lower lobe as in this case).
