Presentation

Right posteromedial knee pain

Patient Data

Age: 30 years
Gender: Female

Right knee joint

There is a large lobulated and septated cystic lesion at the posteromedial aspect of the knee surrounding semimembranosus tendon and displacing gracilis tendon medially. No definite signs of tendinopathy. 

No evidence of meniscal tear.

Both collateral ligaments, anterior and posterior cruciate ligaments are intact.

Intact extensor mechanism.

Normal articular cartilage and bone marrow signal.

Case Discussion

Features of bursitis in the semimembranosus-tibial collateral ligament bursa giving inverted-U appearance. It should be differentiated from pes anserinus bursitis which located more distal and anterior.
