Known "primary" pulmonary hypertension. Sudden shortness of breath ? PE

CTPA

Marked right atrial and ventricular enlargement with reflux of contrast into IVC and hepatic veins. Huge pulmonary outflow tract with large pulmonary arteries showing peripheral "pruning". Note the relatively small sized thoracic aorta.
