Presentation

Right flank palpable mass on physical exam.

Patient Data

Age: 85 years
Gender: Male

A 38 mm fascial defect is seen in the right superior lumbar triangle, and some retroperitoneal fats are herniated through it.

A 6 mm small cyst is noted in the left liver lobe.

Several non-enhanced simple cortical cysts are seen in both kidneys, with a maximum diameter of 88 mm.

Uncomplicated distal colonic diverticulosis is present.

The prostate gland is enlarged.

Degenerative changes such as osteophytes are seen in the lumbar spine. The L5 vertebra is sacralized. Grade I spondyloretrolisthesis of T12 on L1, L1 on L2, and L2 on L3 levels is observed.

Case Discussion

A superior lumbar hernia occurs through the superior lumbar triangle and is more common than inferior lumbar hernias. It may contain a number of intra- or retroperitoneal structures, including the stomach, small or large bowel, mesentery, omentum, spleen and kidney.
