There is a non-displaced supracondylar fracture with associated joint effusion and prominent posterior fat pad. Smooth periosteal reaction is demonstrated overlying the distal humerus which is suggestive of early healing.
Case Discussion
The posterior fat pad sign in pediatric trauma is indicative of a supracondylar fracture. In this case of delayed presentation, further confirmation is provided by the smooth periosteal reaction visible overlying the distal humerus.
