There is a lobulated relatively well-defined soft tissue mass of infra-patellar location, filling the Hoffa fat pad and surrounding the patellar tendon. It displays a low signal on T1WI, high signal on T2WI/STIR with low-signal-intensity linear structures within lesion representing most likely fibrous septa or vascular channels. The postcontrast sequences show an intense and heterogeneous enhancement. There is an associated joint effusion of high signal on T1WI (hemarthrosis).

Case Discussion

MRI features are most consistent with a synovial hemangioma of the knee.
