Tethered cord syndrome with filum terminale lipoma and Tarlov cyst

Presentation

Low back pain.

Patient Data

Age: 70 years
Gender: Female

The MRI sequences demonstrate:

  • low position of the conus medullaris terminating at L4-L5 level

  • thickened filum terminale fixed by a fusiform lipomatous lesion of high signal on both T1 and T2 attenuated on T1 fat-saturated sequence in keeping with a lipoma of the filum terminale

  • large well-defined sacral perineural cyst with spinal canal expansion at S3-S4 level, eliciting a low signal on T1, high signal on T2 and STIR (CSF signals) with Bony remodeling of the sacrum

  • lumbar disc disease with circumferential disc bulge at several levels

  • endometrial thickening (20 mm) of heterogeneous signal on T2 and STIR (needs referral to a gynecologist +/- MRI exam)
