Tethered cord syndrome with filum terminale lipoma and Tarlov cyst

Low back pain.
Patient Data
The MRI sequences demonstrate:
low position of the conus medullaris terminating at L4-L5 level
thickened filum terminale fixed by a fusiform lipomatous lesion of high signal on both T1 and T2 attenuated on T1 fat-saturated sequence in keeping with a lipoma of the filum terminale
large well-defined sacral perineural cyst with spinal canal expansion at S3-S4 level, eliciting a low signal on T1, high signal on T2 and STIR (CSF signals) with Bony remodeling of the sacrum
lumbar disc disease with circumferential disc bulge at several levels
endometrial thickening (20 mm) of heterogeneous signal on T2 and STIR (needs referral to a gynecologist +/- MRI exam)
