Bilateral ophthalmoplegia

Bilateral symmetrical increased thickness of the medial and inferior rectus muscle bellies, sparing their tendons. The affected extraocular muscles display an intermediate signal in T1 and a slightly higher signal than the other muscles in T2.

Case Discussion

This is a case of a patient diagnosed with thyroid-related orbital disease based on the radiological findings and thyroid profile. Thyroid orbitopathy should be differentiated from orbital pseudotumor, which affects the extraocular muscles without sparing their tendon insertions. The pattern of the affected muscles (inferior and medial rectus affected first) is also pathognomonic for thyroid-related orbital disorders.
