Presentation

Acute right iliac fossa pain with leukocytosis.

Patient Data

Age: 30 years
Gender: Male

Retrocecal appendix, its base/ proximal segment looks normal of average diameter and luminal gas content, however, its tip/ distal segment appears abnormally distended measures 13 mm shows an obliterated lumen with a fluid-like content with blurred surrounding fat planes.

Case Discussion

Tip appendicitis is an uncommon variant of acute appendicitis. It is very important to assess the appendix along its course, the base could be normal with a diseased tip and vice versa. This is a little bit difficult in ultrasound however it is slightly easier in CT.
