Vaccination granulomas

Diagnosis almost certain

Presentation

Mild tumefaction of left thigh, pruriginous, recent impetigo due to scratching.

Patient Data

Age: 2 years old
Gender: Female
ultrasound

Areas of echogenic infiltration in the subcutaneous fat of the anterolateral left thigh centered on some hypoechoic nodules, one of which is dominant and more superior, measuring 7 x 5 x 5 mm. A few other small hypoechoic nodules surrounded by fatty infiltration, one less than 6 mm and a few 2 mm in diameter.
 
In the right thigh, no similar hypoechoic nodule seen, but a linear hyperechoic zone of fat 12 mm in length that might correspond to an injection site.

Case Discussion

History, location and imaging findings are typical of post-vaccination granulomas due to allergy to aluminum salts. This is not a contraindication to future vaccinations.
