The caudothalamic groove is an important landmark on when performing neonatal cranial ultrasound.

As the name suggests, it is located between the caudate nucleus and thalamus, and is a shallow groove projecting from the floor of the lateral ventricle. It is approximately at the level of the foramen of Monro, and importantly demarcates the anterior most extent of the choroid plexus. Any echogenicity seen anterior to the groove suggests germinal matrix haemorrhage.
