A 21 mm stone in the gallbladder adjacent to the neck shows a star-shaped pattern within, inferring the Mercedes-Benz sign.
A few small stones are also observed in the gallbladder fundus.
Case Discussion
Mercedes-Benz sign refers to the star-shaped gallstone fissures. Fissures are present in nearly 50% of gallstones and are usually fluid-filled and seen as high signal on T2W MR images. Less than half of these fissured gallstones contain some nitrogen gas observed lucent on x-ray and CT images.
