Os trigonum syndrome

Presentation

Posterior ankle pain. No history of trauma.

Patient Data

Age: 17 years
Gender: Male
mri

Bone marrow edema of the os trigonum, posterior aspect of the talus as well as within the synchondrosis and surrounding soft tissues.

Small effusion within the tendon sheath of the tibialis posterior, flexor digitorum and flexor hallucis longus.

Case Discussion

The clinical presentation and the MRI features are most consistent with os trigonum syndrome which is considered synonymous with posterior ankle impingement syndrome, although the latter may be seen without the presence of an os trigonum.
