Traumatic optic nerve avulsion

Presentation

Trauma, proptosis.

Patient Data

Age: 80 years
Gender: Male

CT Head without contrast

Comminuted left frontal and orbital fractures, with displaced fracture fragments and a small amount of subdural blood. Fractures involving the left frontal sinus, lateral orbital wall, and maxillary sinus. Left globe proptosis with apparent injury of the posterior optic nerve, which appears discontinuous and that it may be avulsed, with retrobulbar blood/thickening. No globe ruptured. Normal appearance of the remainder of the brain parenchyma without other injuries.

Case Discussion

Traumatic left facial injuries with left globe proptosis and discontinuity of the left optic nerve and retrobulbar blood are highly suggestive of optic nerve avulsion.
